Scar was fading nicely, now at 4 months it's gone raised, red and SO itchy. Is this a keloid?

Bit thrown and hoping someone has been here. My scar was doing so well, flat, settling to a pale pink, I was actually proud of it. Now I am just past 4 months and a stretch of it, mostly the bit near the middle, has gone raised and firm and a sort of red purple colour, and the ITCH is unreal, like deep under the skin where I cannot get at it.

Everything I google jumps straight to the word keloid and now I have convinced myself it is going to turn into a big lumpy scar forever and that I somehow caused it. Did I do something wrong? Is a scar that goes backwards like this normal at 4 months or should I be worried? Please tell me your scars did weird things at this stage too.

Mine did EXACTLY this around month 3 to 4. Went lumpy and angry red right when I thought it had settled, and the itch drove me mad at night. Silicone sheets and just leaving it alone (hard, I know) and it calmed right down. By about month 9 it was flat and pale again. It felt like a step backwards at the time but it really was not.

The itch is a healing thing, annoying as it is. Two bits that helped me: silicone (I used the sheets, some people prefer the gel), and keeping it out of the sun completely because sun makes the redness stubborn. The part of mine that stayed raised longest was right over where the tension was, so if yours is worst in the middle that tracks. Do not scratch it whatever you do.

Some general context, because a scar that seems to go backwards around the three to six month mark worries a lot of people and is usually the ordinary course of events. A raised, red or purple, firm and itchy scar in this window is most often what we call hypertrophic scarring, and the key feature is that it stays within the line of the original scar. Scar maturation is slow: redness and raised firmness commonly peak somewhere around three to six months and then soften and fade over roughly twelve to eighteen months. So four months is often close to the loudest the scar will ever be, not the start of a permanent state.

The word people reach for, keloid, is a different thing and much less common: a keloid grows beyond the boundary of the original scar into surrounding skin. What you are describing, worst over the central part where the tension tends to be highest, fits the hypertrophic pattern rather than the keloid one, and tension, individual healing, skin tone and any earlier wound-healing trouble all influence how pronounced it gets. I am describing patterns, not examining you, so I will not label your particular scar, and nobody here should either.

Silicone, sun protection and time are the usual first measures, and your surgeon can assess whether anything more is worth considering for you specifically. What does warrant a prompt review rather than a wait: a scar that is actively spreading beyond its own line, growing lumpier week on week, or breaking down, opening or discharging. Just to add a bloke's data point, mine went raised and red around the same stage and I was sure I had ruined it by getting back to the gym too keen. Backed off the heavy stuff, stuck with silicone, and it flattened out over the following months. It really does look worse right in the middle of the timeline than at the end.

I have started silicone sheets, I am keeping it covered in the sun, and I have booked a review with my surgeon just to have it looked at properly.
