Pregnancy and large weight changes can separate the two vertical rectus abdominis muscles down the midline, a condition called rectus diastasis. The muscle repair, more than the skin removal, is what produces the bent-forward posture and tightness in the first weeks of recovery.

This was the part of my own tummy tuck that nobody had properly explained, and the part that defined the early weeks. What is rectus diastasis?

Rectus diastasis, also called divarication, is a separation of the two vertical rectus abdominis muscles down the midline of the abdomen. Pregnancy and large weight changes stretch the connective tissue between these muscles, and when that tissue is stretched far enough the muscles part, leaving a gap and often a midline bulge. The problem is in the connective tissue, not in the strength of the muscles, which is why it behaves the way it does.

This is a structural change, not simply weak muscles, and both the NHS and the American Society of Plastic Surgeons describe muscle separation as a core reason a full tummy tuck is performed rather than skin removal alone1.

Why exercise cannot close a true gap

Exercise cannot close a true, established rectus diastasis, because the gap is in the connective tissue between the muscles, not in their strength. Core work can strengthen the surrounding muscles and may help very mild cases, but once the connective tissue is significantly stretched, no amount of training brings the separated muscles back together. This is the honest line that took me longest to accept: I trained hard for years against a gap that training was never going to close.

How the muscle is repaired (plication)

The muscle is repaired by plication: the surgeon stitches the stretched muscle sheath back together down the midline, drawing the separated rectus abdominis muscles toward each other to flatten and tighten the abdominal wall. This happens after the skin has been lifted off the abdominal wall and before the excess skin is removed and redraped. In a full abdominoplasty the skin is lifted all the way to the ribs, which lets the surgeon repair a separation running the full length of the abdomen.

The repair is internal and permanent in intent, and it is the step that turns a tummy tuck from a skin operation into core surgery. BAAPS describes muscle plication as a defining feature of the standard abdominoplasty2.

Why the muscle repair drives recovery

The muscle repair, more than the skin removal, is what makes tummy-tuck recovery feel like core surgery. You stand and walk bent forward to protect the repair, and a bent-forward posture for roughly the first 1 to 2 weeks is expected, straightening gradually. There is no core work or heavy lifting for about 6 to 8 weeks to let the repair heal, and a compression garment is usually worn for about 6 weeks.

This is why the timeline is what it is: the tightness across the core, not the scar, sets the pace of the first weeks. Most people feel broadly back to normal by about 6 weeks, with full recovery taking around 3 months3.

Muscle repair and the type of tummy tuck

Whether the muscle is repaired, and how much of it, depends on the type of tummy tuck. A full abdominoplasty and usually a fleur-de-lis repair the separated muscle along its length, because the skin is lifted far enough to reach it. A mini tummy tuck often does not repair the muscle above the navel, because the skin is not lifted that far, so it cannot treat a diastasis that extends above the navel.

Is muscle repair permanent?

The repair is intended to be lasting, but it can be undone. A later pregnancy can re-separate the repaired muscles, and significant weight change can affect the result too, which is why a tummy tuck is best done once you have finished having children and your weight is stable. For most people who have completed their family at a steady weight, the muscle repair holds.
